Spire Group offers individual psychotherapy to adults – with priority given to military members, veterans, and other military-connected individuals (e.g., dependents, civil service employees of the DoD, contractors of the DoD).

We do not currently have providers that see couples but check back in the future for these services.

Note: We do not see children/minors.

Intake Paperwork for New Patients

New patients need to print and complete the linked form below in advance and arrive at your first appointment at its scheduled start time. (This form is fillable, which allows you to type and save your answers.)

If you are unable to print and complete these forms ahead of time, you will need to arrive to your first appointment 45 minutes early to complete the paper packet located in the lobby.

We recognize that the initial paperwork can be time intensive, but we assure you that this is a one-time requirement in service of full disclosure and policy comprehensiveness.

If you have any questions or concerns about any of the items or forms, leave the item(s) in question blank, and address them with your provider at the start of your first appointment.
